LEE ALLAN SMITH OKLAHOMA LEGACY AWARD 2023 NOMINATION FORM CRITERIA: • Nominees must have ambitiously demonstrated volunteer and civic efforts that grow pride in their community; must be well known for engaging the community in their efforts; and have created a legacy of pride for future generations. • Nominations must be received at the Gaylord-Pickens Museum, home of the Oklahoma Hall of Fame, by 5:00 p.m. on Friday, March 24, 2023. ITEMS TO ACCOMPANY NOMINATION FORM: • One-page letter explaining in detail why the nominee should be considered. Letter must also define the legacy created by the individual.
